Baylor Family Medicine

The department of Family and Community Medicine at Baylor College of Medicine has long been known for its comprehensive approach to patient care, service-oriented culture and modern systems, making Baylor Clinic the ideal setting for our private practice services.

Our private practice group, known as Baylor Family Medicine, offers generalist care to adults and children with any type of health concern, with an emphasis on timely appointments to address those concerns – including same-day visits in many cases. Today, we are pleased to offer adult care at Baylor Clinic, while continuing to see both adults and children at our Kirby location.

The Baylor Family Medicine team has always worked closely with Baylor College of Medicine specialists to optimize patient care and treatment. If we can't address the patient's problem, we will quickly identify a specialist who can. That approach is easier today – with more rapid results for our patients – thanks to the collaborative environment and proximity of disciplines within Baylor Clinic.

    We are pleased to offer “Open Access” scheduling, enabling our patients to immediately address their health concerns. Most patients are seen within 1 to 2 days of calling for an appointment.

    We also offer a website allowing patients to request appointments, referrals, lab and X-ray results and refills online.

    Stephen J. Spann, M.D., professor and chair, Family and Community Medicine, received his medical degree from Baylor College of Medicine and completed his residency in family medicine at Duke University Medical Center in Durham, North Carolina. He completed postdoctoral work in clinical epidemiology and health services research at the University of North Carolina in Chapel Hill. Dr. Spann is certified by the American Board of Family Medicine. His primary interests are preventive medicine, diabetes, and medical diagnostic dilemmas. Bilingual/Spanish.

    Maria A. Blazo, M.D., assistant professor. Dr. Blazo earned her medical degree from the University of Texas Health Science Center at San Antonio. She completed her residency in family medicine at the Corpus Christi Family Practice Residency Program and practiced in South Texas for several years prior to starting her academic career. Dr. Blazo recently completed a postdoctoral fellowship at Baylor College of Medicine in clinical genetics. She is certified by the American Board of Family Medicine. Her primary interests are in cancer prevention, genetics, and health promotion for the entire family. Bilingual/Spanish.

    Michael Crouch, M.D., M.S.P.H., associate professor, received his medical degree from Stanford University in California and completed his residency in family medicine at the Medical University of South Carolina in Charleston. He also completed postdoctoral work at the University of Missouri in Columbia. Dr. Crouch is certified by the American Board of Family Medicine. His special interests are obesity, cholesterol, depression, and anxiety.

    John C. Rogers, M.D., M.P.H., professor, received his medical degree from the University of Iowa and completed his residency in family medicine at the University of North Carolina School of Medicine in Chapel Hill, North Carolina. He earned his master's in public health from the University of Washington School of Public Health in Seattle. Dr. Rogers is certified by the American Board of Preventive Medicine and by the American Board of Family Medicine and holds a Certificate of Added Qualification in Sports Medicine. He is particularly interested in prevention in clinic settings and in sports medicine.
